The roof section of the 4-point form records the covering material, roof age, remaining useful life, date of the last roofing permit, and whether the replacement was full or partial. It also records overall condition. Citizens requires a 4-point inspection for applications on properties more than 20 years old.

  • Granules washing off, with bare asphalt or felt showing
  • Look for tabs or tiles that have gone missing, worked loose or developed cracks.
  • Decking that feels soft underfoot or from the attic
  • Stains or active drips on ceilings and the underside of the deck

A wind mitigation inspection is separate. It looks at attic items you cannot see from the ground, such as deck attachment and roof-to-wall connections, and the current form is OIR-B1-1802. It stays valid for up to five years if the structure has not materially changed.
